A Letter from IACP President Wade Siefert, RPh

Fellow IACP Members:

Thank you for the confidence you have entrusted in me! I am both honored and humbled to be your 22nd President. This year we will be facing some of the most daunting challenges our industry has ever faced. Many of you already have stepped up to the plate. Thank you one and all.

Decision
We know that with great challenges, comes great opportunity. So I am reaching out to each and every one of you to be proactive, and seize this opportunity with me, because our Profession needs us. Our Profession is at a tipping point, so when the going gets tough, I draw down from examples set by esteemed colleagues like Past Presidents: Scott Karolchyk, John Herr, Rich Moon, David Nicoletti, Mike Leake and John Preckshot. I admire their decision to lead, and their relentless determination to elevate our Profession, making an indelible impression and opening vast doors of opportunity for the whole of our membership.

AVMA/AHI Update 

IACP’s EVP/CEO David G. Miller, RPh, and VP Government Affairs Sarah Dodge continue ongoing meetings with the American Veterinary Medical Association (AVMA) to determine issues of mutual concern. IACP has confirmed that AVMA will be heading a panel at IACP’s annual Compounders on Capitol Hill as part of an overview of how compounding issues are impacting and being addressed by professional organizations representing prescribers.   

Several weeks ago, IACP was asked by the Animal Health Institute (AHI) to review a draft amendment they are considering proposing to any potential animal drug user fee or authorizing legislation that may move in Congress. AHI, the organization representing the veterinary pharmaceutical industry, would potentially restrict the compounding of certain Active Pharmaceutical Ingredient (API)-based preparations to a “positive” list which the FDA would create and maintain. IACP is opposed to such a list and believes that as written, the proposed AHI concept is clinically unworkable. AVMA and IACP also have discussed this amendment and the veterinary organization understands the difficulties in complying with the current FDA position that the use of APIs for non-food producing animals is prohibited; however, AVMA’s current position is that veterinarians should follow the directions given by the agency and which are spelled out in the 2003 Compliance Policy Guideline on Veterinary Compounding.

Colorado Members: Have you Received a Letter from the Colorado Department of Corrections?

IACP Members practicing in Colorado - you may have received a letter from the Colorado Department of Corrections asking whether you are able to compound sodium thiopental to be used in a lethal injection case.  This letter was addressed in an article from yesterday evening's Denver Post.  Click here to read the Denver Post story.

IACP and the Colorado Pharmacists Society (CPS) spoke on Wednesday morning, March 13, 2013, about this letter.  CPS has determined that the Department of Corrections did not contact the state's Board of Pharmacy prior to issuing the letter.  It is not yet known whether compounding or dispensing any medication which the pharmacist knows will cause harm is legal within the state.  CPS also is notifying its members of that as IACP is doing as well.

There are several additional business issues to consider when approached to compound a medication used for executions that are separate from the ethical and legal ones. First, make sure your professional liability insurance provides coverage for such an action.  Second, consider the potential security implications to your practice, your employees and your families. Given that the death penalty is a controversial and very high-profile issue, having your pharmacy known as the source of the lethal medicine may bring unwanted attention -- media, picketing by death penalty opponents, threats, etc.  IACP and CPS will continue to keep you informed on this as we gain additional information.

Can you Compound 17-P Hydroxyprogesterone in Iowa?

Several Iowa IACP members have contacted our offices to ask about an article that appeared in the Board of Pharmacy's most recent newsletter about the compounding of 17-P.  Some have interpreted the Board's article to mean that compounding of 17-P hydroxyprogesterone is not permitted by Iowa pharmacy permit holders.  That is not correct.

In fact, at the beginning of the year, the Iowa Medicaid department issued an extensive guidance letter to providers that explained exactly how billing for Makena® or the compounded 17-P is to be handled. Click here to view the letter.
